Service agreement

A service agreement is a document a participant, or their nominee, signs with a provider before supports start. It sets out what will be delivered, how often, at what price, and under what cancellation or notice terms.

A service agreement is not the same as a service booking, which is the record of confirmed supports and funding held inside the NDIA’s own systems for agency-managed arrangements.

A service agreement is optional in some circumstances and expected in others, and its terms sit alongside the NDIS Code of Conduct rather than replacing it.
