An NDIS invoice needs a specific set of fields so a plan manager or the agency can match it to a plan and pay it without back and forth. This page sets out those fields and will offer ready-made templates in Word, Excel, and PDF. The downloads are being prepared; the field checklist below lets you build a compliant invoice today.
Why the fields matter
A plan manager pays from a plan, so every invoice has to identify the participant, the provider, the support delivered, and the amount. A missing field is the most common reason an invoice bounces back for a fix.
Support item codes are the part providers most often leave off. The code tells the plan manager exactly which support a line relates to, which is why it belongs on every line alongside a plain description.
Building an invoice from the checklist
The field list below covers what a plan manager looks for. Put each field on your invoice, one line per support delivered, and show the GST treatment clearly on each line.
When the templates are ready they will carry these fields already laid out, with a version for providers who invoice the agency directly and one for those who invoice a plan manager. Until then, the checklist is the same information in list form.
