A service agreement records what a provider will deliver, at what price, and on what terms. It is agreed between the participant (or their representative) and the provider before supports begin, and it is where the cancellation and travel rules are written down so an invoice never comes as a surprise.

Sole traders, support workers, and larger providers all use one. The template below gives you the sections to fill in; the guide underneath explains the terms the sector expects to see.

What a service agreement should cover

There is no single mandated form, but a workable agreement records the parties, the supports and agreed prices, and the terms below. Several of these terms come straight from the NDIS Pricing Arrangements, so writing them to match keeps claims clean.

  • Participant name, NDIS number, and representative or nominee if any
  • Provider name, ABN, and contact details
  • How the plan is managed (self-managed, plan-managed, or NDIA-managed)
  • The supports to be delivered, each with its support item code and agreed price
  • The agreement start and end dates
  • How provider travel time and travel costs are charged
  • The cancellation terms (notice period, and what can be billed for a late cancellation)
  • How the provider is paid, and how invoices are issued
  • How the agreement can be changed or ended, and by whom
  • Signatures and dates from both parties

Rule What the official rules say
Cancellation notice A short notice cancellation applies with less than 7 days notice for disability support worker supports, or less than 2 clear business days for other supports (or a no-show).
Cancellation charge Where the provider could not find other billable work, up to 100% of the agreed fee for the activity may be claimed; the provider must pay the worker for the time and may choose to waive the fee.
Provider travel time Up to 30 minutes each way per worker in MMM 1-3 areas, up to 60 minutes in MMM 4-5, and no time limit in remote and very remote areas, plus non-labour vehicle costs.

How your plan is managed - self-managed, plan-managed, or agency-managed - changes who you can buy from and how invoices are paid. Your plan documents state which applies to you.

Agency-managed participants generally need to purchase through registered NDIS providers. Self-managed and plan-managed participants can buy from any suitable supplier.

How to use it

Sole traders and support workers

Keep it short and clear. Record your name and ABN as the provider, the agreed hourly supports, and the cancellation notice you need. As a sole trader you are not required to pay a separate worker, so note that in your cancellation terms.

Larger providers

Add the detail your operations need: which team delivers what, how travel is split across participants, and your complaints and privacy processes. The core price, travel, and cancellation terms stay the same.

Questions people ask

Is a service agreement required under the NDIS?

A written service agreement is strongly encouraged and, for some supports, expected. It protects both sides by recording the price, the travel and cancellation terms, and what each party agrees to do. Putting the terms in writing also makes invoices easier for a plan manager to check and pay.

What cancellation terms go in the agreement?

Record the notice period and what happens for a late cancellation or no-show. The Pricing Arrangements set the notice windows and the most that can be billed. Writing these into the agreement means both the participant and the provider know the terms before any cancellation happens.

Can a sole trader use this template?

Yes. The template suits sole traders, support workers, and larger providers alike. Fill in your name and ABN as the provider, list the supports and agreed prices, and set the cancellation notice you need. The variant notes above cover the small differences for each.

Does the agreement set the price a provider charges?

The agreement records the price both sides agree to. For managed arrangements it sits at or below the price limit in the current Pricing Schedule; self-managed participants can agree a different price with the provider. The agreed figure goes in the supports table so invoices match.
