How to use it
Support coordinators and providers
Tie every request to a goal and to evidence. A functional capacity assessment or therapy report attached to the relevant request does more than a general statement of need.
Participants and families
Use the report to say what you want the next plan to help with, in your own words. Bring it to the reassessment alongside any reports that back up what you are asking for.

What is a plan reassessment?
It is the review where the NDIA looks at how the current plan went and works out the next one. This report helps you prepare by gathering goals, progress, and the supports needed going forward. The NDIA decides what the plan includes; the report is preparation, not the outcome.
What evidence helps at a review?
Evidence that connects a request to a goal: functional capacity assessments, therapy reports, and letters from treating professionals. Attaching the right evidence to each request, rather than a general statement, gives the reassessment a clear basis for the supports you are asking about.
Who prepares the report?
A support coordinator, provider, or the participant and their family can prepare it, often together. Sign-off from the participant keeps it grounded in what they want. It is a shared preparation document, so the more the participant voice is in it, the more useful it is.
